How does Nebipro 5 work?

Nebivolol, the active ingredient in Nebipro, is a beta-adrenergic receptor blocker that reduces heart rate, myocardial contractility, and renin activity. It also promotes vasodilation and decreases peripheral vascular resistance.

Indication of Nebipro 5

Nebipro 5 works specifically on the heart by slowing down the heart rate and making the heart more efficient at pumping blood around the body. This results in a lowering of the blood pressure.

Pharmacology

Nebivolol is a β-adrenergic receptor blocking agent that inhibits both β1 and β2 adrenergic receptors, though it lacks intrinsic sympathomimetic and membrane stabilizing activity at therapeutic doses. It primarily affects the cardiovascular system by:

• Reducing heart rate
• Decreasing myocardial contractility
• Reducing sympathetic nervous system outflow
• Suppressing renin activity
• Promoting vasodilation, which lowers peripheral vascular resistance

Pharmacokinetics:

• Metabolism: Nebivolol is metabolized via glucuronidation and hydroxylation, primarily by the CYP2D6 enzyme. The active isomer (d-nebivolol) has an effective half-life of 12-19 hours, depending on the individual’s metabolic capacity (extensive or poor metabolizers).
• Absorption: It is well-absorbed, with peak plasma concentrations occurring within 1.5 to 4 hours. Food does not affect absorption.
• Protein Binding: Nebivolol binds to plasma proteins at approximately 98%, mainly to albumin.
• Excretion: After oral administration, about 38-67% of the drug is excreted in urine and the rest in feces, depending on the individual's metabolic status.

Dosage & Administration of Nebipro 5

• The initial recommended dose is 5 mg once daily. The dose can be increased at intervals of 2 weeks, if necessary, up to a maximum of 40 mg per day. For elderly patients over 65, the starting dose is 2.5 mg once daily, which may be increased to 5 mg if needed.
• The initial dose for heart failure is 1.25 mg once daily, which may be doubled every 1-2 weeks if tolerated, up to a maximum dose of 10 mg daily.
• For patients with hepatic impairment, the starting dose is 2.5 mg per day, with cautious increases if necessary. In patients with renal impairment (CrCl <30 mL/min), the initial dose is 2.5 mg daily, which may be increased to 5 mg daily if required.
